IRB Closure Report

To maintain an accurate account of active studies on campus, PIs are strongly encouraged to complete a Closure Report once all human subject research activities have been completed for each study.

Close your study

A protocol is eligible for closure if ALL of the following are true:

  • Data collection is complete
  • There is no more participant contact (including follow-up interviews, calls, surveys)
  • The only research activity remaining is analysis of de-identified data
    • Information is identifiable if participants can be identified directly or indirectly through identifiers linked to the participants. This includes any key linking participants to pseudonyms/study IDs.
  • A Closure Report should be filed within 30 days of IRB-approved protocol completion. The IRB does not recommend allowing IRB-approved protocols to expire or be administratively closed by the IRB office, as this does not bring proper completion to protocol history. If you would like an extension, please reach out to the IRB office.

To close your study, you will need to complete the Closure Application Form and submit it in a new package. No other form is required. Co-investigators or IRB Liaisons do not need to sign the package.
